Healthy Choices Benefit Plans assist clients formulate and implement benefit strategies, which reflect corporate objectives, meet financial requirements and satisfy employee needs through cost savings, networks, reinsurance, case management, and pharmacy vendors.

DENTAL PRODUCTS
Healthy Choices Dental Plans offer competitive products that are designed to provide and promote preventive care regular check ups along with basic and major care services provided. Employers can select from a variety of plan designs and options, and employers with 10 or more employees can offer 3 dental plans to their employees.

You can select from a preventive care with limited basic services program to a preventive care service program with basic, major and orthodontia coverage. Benefits can be offered on a group voluntary basis. Employers with 4 or more employees are eligible to enroll in our plans. Our dental products have participation requirements.

VISION PRODUCTS
Healthy Choices Vision Plans are wellness vision programs for employers and employees. Whether you as the employer want to complete your total benefit package, add additional benefits or offer only a vision program to your employees, we offer flexible plan designs, ease of administration, national provider network and customer service to you and your employees.

You can select from different co-payment options for your annual eye exams, as well as materials co-payment for lenses. We offer co-payments for frames and contacts or select a benefit paid in full up to an annual allowance. Additional discounts are available on optional benefits at our Value Added Providers (VA) and LASIK surgery discounts are available through TLC Vision.

Enrollment requires a minimum of 5 employees or 20% of the group.
